OVERVIEW
Excellent Support Care Corp. will serve Medicaid-eligible individuals struggling with Chronic Health Conditions, Including Asthma, Diabetes, Heart Disease, HIV/AIDS, Hypertension, Obesity, Mental Health Illness, and Substance Abuse. Utilizing a client-centered, family-focused approach, our Care Management Service Model works to remove barriers and Improve Access to Primary and Behavioral Health Care. Collaborating with the individual, Medical and Behavioral Health Providers, and other Community-Based Network Providers, we address the individual’s needs and goals in a comprehensive manner. Our Care Management team will coordinate these Individualized Services (Housing, Entitlement Benefits, Medical Case Conferences, Health Education/Promotion, Advocacy, etc.) to ensure our community members stay Healthy and Reduce Emergency room visits and Inpatient Services. Our Services are culturally competent. Our future goal is to enable our clients to have the option to meet with Spanish and Creole Speaking Care Managers to address Language Barriers and to Advocate and Coordinate the delivery of Supportive Care that addresses the client’s quality of life as well as his or her Needs and Aspirations.
Our Benefits Advocacy Case Management model serves as a bridge to help Clients learn to Navigate the Health Care and Support System and gain the knowledge and Skills necessary to effectively Self-Manage their Care. Through these services, ESC helps Clients gain access to Maintain Primary Care, Manage Medications and Treatment Regimens, build strong Support Systems, and find safe and Affordable Housing. The fundamental aspects of Care Coordination are to Develop an Effective and Sustainable Processes to ensure families are Appropriately Referred and to ensure they can access a wide range of Essential Resources. We use Medical Case Conferences to remove the Barriers that Prevent Retention in Care and Treatment. We use it to also help Support Clients’ entry into the full continuum of care that is available.
Our staff provides information and assessments to help individuals and families living in Queens determine if they may be eligible to apply for food stamps. We work to increase participation in the School Breakfast Program and the Summer Food Service Program at our Community Schools. To ensure that we reach our community members, we Provide Services in our offices and Partner Schools and Agencies and conduct home visits. There are many people in the country eligible for these Benefits who are unaware that they are eligible and have not applied to receive Food Stamps; the goal of this program is to provide Nutritional Education, Community outreach, Information, and Assistance for Enrollment into the Supplemental Nutritional Assistance Program (SNAP) for to individuals and families in need of Nutritional Services.
The best way to reduce homelessness is to prevent it in the first place. Undiagnosed mental illness and/or physical disability is an invisible anchor for many homeless people, making it nearly impossible for them to get necessary help and find a way out of homelessness. Having a stable, affordable home is essential for both individuals and society. A safe and decent home allows parents to maintain jobs, children to stay in the same schools, and the local economy to be supported. Having a home of their own empowers these men and women who have shown such resilience despite their disabilities to become active agents in their own welfare and livelihood for the rest of their lives. Having a home also underscores the inherent dignity of the persons who live there.
Navigating the NYC shelter system can be a full-time job in itself, and too many are wrongly turned away for lack of proper documentation or because shelter workers believe applicants have somewhere else to stay. Our care management team uses their exceptional knowledge in homelessness and the complex details of the NYC shelter system to ensure that everyone who needs shelter gets the proper help, which saves tax payers over $60, 000 per year in shelter costs.
ESC’s goal is to develop and maintain strong relationships with community partners, including health care providers, community-based organizations, faith-based organizations, and legislators to develop the infrastructure that ensures ESC program participants are receiving quality services. Also equally important is developing and maintaining partnerships with health home leads, contract managers, and MCOs.
